Problem definition: We consider two problems faced by an operating room (OR) manager: (1) how many baseline (core) staff to hire for OR suites, and (2) how to schedule surgery requests that arrive one by one. The OR manager has access to historical case count and case length data, and needs to balance the fixed cost of baseline staff and variable cost of overtime, while satisfying surgeons’ preferences. Academic/practical relevance: ORs are costly to operate and generate about 70% of hospitals’ revenues from surgical operations and subsequent hospitalizations. Because hospitals are increasingly under pressure to reduce costs, it is important to make staffing and scheduling decisions in an optimal manner. Also, hospitals need to leverage data when developing algorithmic solutions, and model tradeoffs between staffing costs and surgeons’ preferences. Nurse Scheduling Problem

In order to reduce cost and to optimize the use of resources, hospitals are prompted to regroup facilities and human resources, especially in the surgical suite. This work focuses on nurse scheduling problem in an Albanian public hospital, where the anaesthesiology nurses constitute one of the most shared resources. They work in a cross way over surgical specialities and assume various activities. The assignment of working shifts to nurses over a period of several days is a difficult and time-consuming task, which is usually handled manually by the head nurses. The principle of sharing resources within larger surgical suite, in order to reduce the costs, makes more complex the nurses scheduling problem. This reinforces the need of computer-aided tools for this particularly complicated task. It will be computed by Lingo Modeling Language and Optimizer [1]. Objective: To find out reasons for cancellation of operation on the day of surgeryin the Surgical Department of Civil Hospital Karachi. Design: Observational study. Setting &Period: All operation theatre bookings of the Civil Hospital Surgical Department from July 2010 toOctober 2010 have been studied by using the ‘daily operating theatre lists’. Frequency ofcancellation, type of surgeries cancelled and their reasons studied. Results: Out of total 455cases, 97 (21%) surgical operations were cancelled on the day of surgery, highlighting patients’non-compliance as a major contributing reason, followed by the lack of operating room time andother causes. Among the less contributing factors were patient expiry on table, failure toadminister anesthesia and delay in operation due to exams (engagement of patients inundergraduate exam in the college).
